Same or a bit larger than the bore.

a cylinder so tight it was squeezing my bullets down smaller than the bore. An ancient .38/40 with 0.398" throats, best I could tell back then. The bore was 0.403". It shot all over the place and leaded cast bullets badly. An extreme case.

Other than that I have occasionally had a small issue where the cylinder throats were a thou too tight to allow the projectiles I wanted to use. Either you don't use that diameter/type projectile or quietly mod it yourself.

Too many armchair experts get guys all whipped up into a frenzy over the teeny dimensions. If it shoots the ammo you load accurately don't fret or obsess over it too much.

As mentioned, shooting wild groups or key-holing is another matter. As is, the ammo not fitting the gun. You just fix it. It's all part of the fun and is so satisfying to have victory over an extreme problem.

as a .224 and the 38 special, according to which company. Colt likes .356, S&W likes .357 (again from what I have read).
I don't worry too much about the cylinder throats, at least as much as some. The reamer is usually 6 to 8" long and the front of it cuts the cylinder throats at the same time the rear cuts the chambers. If the reamer is fairly new and to specs, as they normally are, then the throats of the cylinder should be fine (my opinion only). Once our reamers are beginning to get dull I throw them out. I never resharpen them as that takes a few thou off the size and they cut too tight a throat. Dave Manson, my reamer guru, is very picky about his reamers and that is one of the reasons I stick with him. I have yet to find fault with any of his reamers and I have been using his reamers since 1985.
